For your convenience, we offer several forms for your use:
Permission Slip - For more world experience and just plain fun, we take the students outside of the walls of the church occassionally. A Permission Slip is required for each of these events, which are given to students ahead of time. If you need another one, you can click on "Permission Slip" to the left to print one.
Photo Waiver - In order for us to use you or your child's picture or video, we need to have a signed waiver stating that you give your approval. Please click on "Photo/Video Waiver" to the left to print one. This may be turned in at the church office.
